本人授權健康殿堂集團有限公司及 Gutolution Limited 進行實驗室檢測，授權收集樣本以進行檢測，並有權根據《個人資料（私隱）條例》獲得保護。我確認並同意以下事項：

• 資訊準確性：我理解檢測結果的準確性取決於我提供的資訊的完整性和正確性（例如性別、年齡、健康問卷等）。

• 對檢測的理解：我了解檢測的目的、性質和限制，並明白結果僅供參考，並不構成臨床診斷。

• 樣本收集政策：我理解如果我的樣本不適合測試，我可能需要提供新的樣本。如收集樣本超過兩次以上仍未達標，可能會產生額外收費。如果我選擇不提供新樣本，我將不會獲得退款。

• 或預防以及治療任何疾病。

• 數據保留：我理解上述公司可能會保留數據及任何多餘的樣本以作研究用途。

• 個人資料使用：我確認我的個人資料將僅用於檢測相關目的，並且我可能會收到有關推廣和其他市場活動的通信。我有權隨時通知上述公司停止接收此類通信。

I authorize Health Care Signature Group Limited and Gutolution Limited to conduct laboratory tests, to collect samples for testing, and have the right to protection under the Personal Data (Privacy) Ordinance. I confirm and agree to the following:

• Accuracy of Information: I understand that the accuracy of the test results depends on the completeness and correctness of the information I provide (e.g., gender, age, health questionnaire, etc.).

• Understanding of Testing: I am aware of the purpose, nature, and limitations of the test and understand that the results are for reference only and do not constitute a clinical diagnosis.

• Sample Collection Policy: I understand that if my sample is unsuitable for testing, I may need to provide a new sample. If the sample fails to meet standards after more than two collection attempts, additional charges may apply. If I choose not to provide a new sample, I will not be eligible for a refund.

• nor are they intended for the prevention or treatment of any disease.

• Data Retention: I understand that the aforementioned companies may retain data and any excess samples for research purposes.

• Use of Personal Data: I confirm that my personal data will only be used for testing-related purposes, and I may receive communications regarding promotions and other marketing activities. I have the right to notify the aforementioned companies at any time to opt out of such communications.

健康殿堂集團有限公司按《個人資料（私隱）條例》的規定，處理及妥善儲存所提及的個人資料，為保障資料當事人的利益，本公司只收集當事人提供的個人資料作檢測申請之用。

本公司可能運用所提供的個人資料（包括姓名、電話、傳真、電郵及郵寄地址），以便日後與你通訊、用於活動或研討會邀請、收集意見，或作有關的宣傳與推廣等用途。

你可以隨時要求本公司停止使用你的個人資料作上述推廣之用途，費用全免。

Health Care Signature Group Limited complies with the provisions of the Personal Data (Privacy) Ordinance in processing and securely storing the mentioned personal data.

To protect the interests of the data subjects, the company only collects the personal data provided by the individuals for the purpose of test applications.

The provided personal data (including name, phone number, fax, email, and mailing address) may be used for future communications, event/seminar invitations, opinion collection, or related promotional and marketing purposes.

You may request the company to cease using your personal data for the aforementioned promotional purposes at any time, free of charge.
